Recently, the infant children of actor Dennis Quaid were involved in a hearth scare when they were accidentally given the blood-thinning medication heparin in the adult dose, not the pediatric dose. Quaid and his wife discussed their ordeal on the CBS news program 60 Minutes.


Who do you think is responsible? The doctor for not clearly communicating the medication order? The nurse for not reading the label of the medication container carefully? The pharmaceutical for designing similar containers for the pediatric and adult doses of heparin? What do you think?
